Please supply the following information from the company’s Pesticide Business license. If the company does not have a BU#, please write “NEW” in that field. You may indicate “unknown” for BU# and provide the business license information to us ASAP but no later than 3 weeks prior to the class date. Questions about business licensing should be directed to the PA Department of Agriculture at 724-832-1073. Without a BU# to which to link your pesticide certification to, you will not be permitted to “apply pesticides”.

Cancellation Policy *

You may request, in writing, to cancel your registration up to 10 business days prior to your class start date and receive a refund less a materials fee of $150 for Pesticide. You may request, in writing, to send a substitute registrant at any time for the CPO. Substitution within two weeks of the Pesticide exam requires approval by the PA Department of Agriculture. Due to the strict capacity limits of pesticide exam sessions, we are unable to accommodate date changes. If you don’t show up for your class, you’ll forfeit your payment.

State issued photo ID is required for testing. Fee includes Text Books, Review, Core and Category 24 tests and lunch.
